A: Only a few items are required to sign-up for our program.
Please pick up a brochure and forms from either the cafeteria during program hours, or from the Niu Valley Middle School or Moanalua Middle School Office. We are available after-school to answer any of your questions and address any concerns you may have. You may also reach us by phone and email with inquiries. In order for your child to attend our program, we require an application, a registration agreement, and payment of the monthly fee. These three items may be submitted the day your child will begin attending, or earlier. Downloadable copies of the application and registration agreement for our programs may be found under the ‘Forms & Applications’ portion of our website, and can also be emailed upon request.
